What it really is a journey that leads you to an item that you can forge from the organs of uber-powerful Diablo III boss tag-teams. Like any journey, this one begins with the first step. To begin your journey all you need to do is find and kill a Keywarden. The Keywardens in the first three acts drop a key, and the Keywarden that appears in Act IV drops the crafting plan for the Infernal Machine.
For more information on the Keywardens and the drop rates of the keys click here. Once you gather all three keys and the crafting plans you can then take them all to the blacksmith. Right click on the plans to teach them to blacksmith and the Infernal Machine will appear on the Weapons tab under the new Portal Device heading look under P.
The Infernal Machine costs 12, gold to craft. Once crafted the Infernal Machine takes up one item slot in your inventory the size of a ring. To be fair you can use one machine to open one of the three portals that transport you to a demonic realm, but it does not make sense to do that. When you use your first Infernal Machine to open one of the three portals it will randomly open one of the three possible portals.
You can enter that portal fight the uber bosses and hopefully get the organ piece you need. If you leave the game for the day or a meal and you end your game session that one portal you opened will close. When you start up a new game session and you use your second Infernal Machine, what if it randomly opens Well that is hardly ideal since you already killed that boss pair and you don't need that organ again!
To prevent this from happening what you should do is use three Infernal Machines to open all three portals at the same time. Make sure to plan out your game session ahead of time so that you will have enough time to enter all three portals and kill all three boss pairs. When you do this, not a single Machine is wasted on a portal that you didn't want.
Before you enter your first portal you and any friends that are playing with you must have a full five stacks of Nephalem Valor. You will not get an organ if you don't have a full five stack! The portal room is in Act I and you should start the game with the Return to New Tristram quest activated. The Cemetery of the Forsaken is a good spot to find and kill elite packs because there are always at least three dungeons that spawn there and one of them should have a resplendant chest that you give you and easy Nephalem stack.
The portals can only be opened inside the Heretic's Abode in New Tristam. It is a shack with a breakable door located behind the NPC that can heal you. When you are ready to step through your portals it is a good idea to have three friends or strangers with you.
If you want a high organ drop rate chance you will need to select a high Monster Power , but you still need to be able to kill the boss pair within 8 minutes. The uber bosses have an 8 minute enrage timer which means after 8 minutes they will become invincible and they will kill your whole party. During the fight you can revive each other as long as one person stays alive.
If all of you die, you will be kicked out the demonic realm and you will have to re-enter the still opened portal to try again.
